Liposuction (lipoplasty) is usually adequate for treating
excess fat accumulation in the abdomen, but in those patients
who have significant skin laxity and/or muscle separation
in the middle of the abdomen, an abdominoplasty, or tummy
tuck, is sometimes needed. Significant skin laxity most commonly
occurs following multiple pregnancies, or after dramatic weight
loss. Once the skin tone has been lost, the only way to correct
the laxity is with skin excision, which is done in an abdominoplasty
through an incision in the lower abdomen that is covered by
underwear and bathing suits. In addition, these patients may
also have a separation of the rectus abdominus muscles in
the middle of the abdomen (the "six pack" muscles), especially
after multiple pregnancies. The abdominoplasty also includes
sewing the muscles back together again in the middle, which
strengthens the abdominal wall, supports the lower back, and
narrows the waistline at the same time by tightening the muscle
layer. The muscle closure and the skin closure are both done
under some tension in order to maximize the improvement, but
that also may result in significant scarring in the lower
abdomen. Patients with appropriate indications feel that the
tradeoff is a good one, since the improvement in the contour
and firmness of the abdomen is quite dramatic.
After the surgery, a compressive garment is worn for one
month, much as is done following liposuction, but the recovery
following the abdominoplasty is more significant, with moderate
pain for the first several days, and limited activities for
the first couple of weeks.
