The eyes are one of the first areas to show signs of aging
in many people, both in the upper eyelids with the sagging
skin, and in the lower eyelids with the bag formation and
wrinkling. Both men and women are concerned about this "tired"
look, which friends and associates often comment on. Since
the eyes are the proper focus in conversation, it is natural
that the changes in this area are more noticeable, and are
frequently the first problem that brings patients in to see
Dr. McGuire. In some cases, sagging of the eyebrows is also
a factor, often giving patients an unhappy and tired appearance.
Surgery to elevate the eyebrows and reduce the frown lines
can be done with the upper eyelid surgery through the same
incision, and is discussed in a separate section on this site.
The modern approach to the changes that occur in the upper
and lower eyelids has dramatically improved the results, avoiding
the artificial and distorted shape of the eyes that was sometimes
seen with older techniques. Surgery on the upper eyelids is
done through the crease line, so that the scar is very minimal
and undetectable. The excess skin is reduced, and sometimes
surgery is done on the eyebrows and frown lines at the same
time. In the lower eyelids, in most cases, the surgery is
now done through the inside of the eyelid, avoiding any external
scar at all, and reducing the pull on the lower eyelid that
often resulted in distortion. The bags of the lower eyelid
are actually fatty tissue, which over time bulges outward
through a weakening of the membrane that holds the fat in
place during youth. The older procedures removed most of this
fat, and often left a sunken appearance to the lower eyelid
that was equally undesirable and often artificial. Today,
the fatty tissue may be reduced, but it is also used to fill
in the depression just below the bags to give a very smooth
contour to the lower eyelid that is much more natural and
youthful in appearance. If skin wrinkling in the lower eyelids
is a problem as well, this can often be treated at the same
time with a mild resurfacing procedure of the eyelid skin,
using either a laser or a chemical. Both upper and lower eyelids
can be treated at the same time under the same anesthesia,
which is a light general anesthetic. The modern light general
anesthesia used is extremely safe, effective, and without
the subsequent problems sometimes seen in the past. After
surgery, there is some bruising and swelling for up to a week,
but the pain is minimal, and most patients are back to normal
activities and work within a week. The result is a more vibrant
appearance, as well as more youthful, and the results are
long-lasting.
