Frown lines give patients an unhappy or angry
appearance that others comment on, and are often associated
with sagging of the eyebrows that give a tired or aged appearance.
Many patients come to Dr. McGuire complaining that their coworkers
tell them that they look tired or angry, and they're not,
so they are anxious to get rid of this unwanted expression,
and achieve a more vibrant, youthful appearance. Often, these
same patients have upper eyelid skin excess that further contributes
to the tired appearance, and today both problems can be treated
at the same time through the same incision.
In the past, incisions were made in the scalp or hairline
in an attempt to indirectly reduce from above the muscles
responsible for drooping eyebrows and frown lines. These procedures
were known as forehead lifts, but there were numerous problems
associated with this procedure, which Dr. McGuire did many
years ago, but has not done for the past twelve years. Forehead
lifts left significant scars in the scalp or hairline, often
associated with bald spots and permanent numbness in the scalp.
The result was often a "startled" expression, with the eyebrows
overly lifted, and the result was not long-lasting since the
muscles that pull the brows down and create the frown lines
were not effectively reduced. More recently, Dr. McGuire,
and a number of other leading plastic surgeons in the
United States, analyzed the problems with forehead lifts,
and developed a procedure that can be done through the same
upper eyelid incision that is used to reduce the sagging skin
of the eyelid. Through this incision, simply lifting the skin
allows direct and precise exposure of the muscles responsible
for the frown lines and for pulling the eyebrows down. These
muscles, known collectively as the glabellar muscles, are
the same muscles that are paralyzed when Botox® is used to
reduce the frown lines. The Botox® effect is only temporary,
however, lasting three to four months, and many patients eventually
tire of the repeated injections and associated costs. The
surgery to reduce these muscles is permanent in its effect,
and still allows eyebrow motion and expression, except for
forceful frowning. The brow is released with the reduction
of the muscles and with the surgery across the upper eyelid,
and rises more naturally, avoiding the startled expression.
The result is permanent, and gives a more vibrant, rested,
and youthful appearance, opening the eyes, and improving the
shape of the brow as well. The surgery is frequently done
in conjunction with upper eyelid skin reduction surgery, which
is described in another section. The result is virtually permanent,
and the results have been some of the most beautiful and dramatic,
with all patients very satisfied.
Dr. McGuire is one of the very few plastic surgeons performing
this surgery, and certainly has one of the largest and longest
experiences with this dramatic rejuvenation procedure.
